Description:
Donnie Werner has discovered a vulnerability in phpFormGenerator, which can be exploited by malicious people to compromise a vulnerable system.

The application allows people to create custom forms. The vulnerability is caused due to an error in the handling of file uploads for forms created with the "File Upload" field type. This can be exploited to upload arbitrary files to a location inside the web root (e.g. a malicious PHP script).

The vulnerability has been confirmed in version 2.09c. Other versions may also be affected.

Solution:
Restrict access to create forms and do not create forms with the file upload control.
