I just installed all Microsoft updates successfully. However, checking the updates in detail, this update reported failed. Rebooted, ran Microsoft update again but doesn't offer the update again. PSI confirms v. 4.1.10111.0 as patched. What can I do?

I had the same problem on a Windows 7 machine and then I realised I hadn't updated to Sliverlight 5 on it.

Microsoft have identified a metadata issue with this update and it is now being held back from automatic update until resolved.
